Home and Community-Based Services (HCBS) offers opportunities to build essential skills in the comfort of your home or community. This is done by setting personalized goals within your habilitation plan, set by you and/or your family/relative. Goals could relate to personal care, social skills, household chores, community integration, budgeting, cooking, and more.

Services are provided one-on-one, allowing both the staff and the person they are working with to learn from one another. You are matched with HCBS staff based on your specific goals and needs. As you gain and develop skills, staffing may change to effectively support your growth.

Springbrook currently supports more than 220 people enrolled in respite services and community habilitation across Chenango, Broome, Delaware, and Otsego counties.
